As you prepare to move to Ivy Living, our team will provide you with a form called a Physician’s Report. Your primary care physician will use this form to provide us with any medical information that will help us determine how we can best support you. Along with the Physician’s Report, you will meet with our Health Service Director for a level of care assessment. During this meeting, you and the Health Service Director will discuss your needs and create a plan of care.
The goal at each Ivy Living community is for our residents to be able to continue to live with us as their needs change. Our care plans are flexible to meet your needs and can be updated any time it is necessary.
