On-Site Primary & Specialist Healthcare – In partnership with Pine Park Health, Ivy Park at San Tomas provides in-residence medical care designed for comfort, convenience, and continuity. Residents have access to a wide range of on-site services, including primary care and specialist visits such as podiatry, dentistry, optometry, audiology, ENT, and therapeutic massage. Lab work can be completed on-site, eliminating the need to travel for routine care.

On-Site Physical & Occupational Therapy – Our on-site rehabilitation center makes therapy convenient and seamless. We offer both physical and occupational therapy steps from your front door—no need to arrange transportation. Our team coordinates appointments and care to ensure a smooth, supportive recovery process right within the comfort of your community.

Daily Nursing Coverage – Licensed support and coordination for residents with needs such as diabetes management, catheters, injections, wound care, and more.

Certified Dementia Practitioner (CDP) Site – Every caregiver at San Tomas is a Certified Dementia Practitioner, specially trained to support residents living with memory loss. We’re also proud to serve as an official CDP training site, demonstrating our commitment to excellence in dementia care and ongoing professional development.

Opening Minds Through Art (OMA) – We’re thrilled to partner with Opening Minds Through Art (OMA), a Stanford-affiliated, evidence-based program that fosters creativity, connection, and joy. This award-winning initiative brings intergenerational engagement to our community, enriching the lives of our residents through meaningful artistic expression.

Support Beyond Care: Family Connection & Compassion – We offer a professionally facilitated Family Support Group, which meets monthly and provides a consistent space for loved ones to share, connect, and learn. This ongoing support empowers families throughout their care journey.
